By completing this form and saying 'yes' to the consent field you confirm that you have read, understood and will comply with the following:
- Sleaford Barge Netball Club Code of Conduct (available on the website).
- Sleaford Barge Netball Club use of WhatsApp is in place to support organisation of training, matches and club events. I understand that by being a player, I will be added to a team group and comply with the usage policy for the use of WhatsApp (available on the website).
- Sleaford Barge Netball Club will take photographs and videos for the use in training, advertising of events and PR through club website and social media; therefore, I am consenting to the use of photographs and the usage of video recordings.
- Sleaford Barge Netball Club GDPR Privacy Policy (available on the website). I understand that in order to be an active member of Sleaford Barge Netball Club i.e., attends matches, training and events; that personal data (as stated within the Privacy Policy) will need to he held securely by the club in order to comply with the GDPR Policy. I also understand that my personal details will be used to affiliate me to England Netball, and my contact details may be used by the Club for Club business.

If this form has been completed on behalf of a junior member (under 18), you as the Parent/Guardian are confirming that you have read and understood the following:
- I agree to my son/daughter/child in my care taking part in the activities of the club.
- I agree to my mobile number being used in team WhatsApp groups and understand that these groups will be monitored to ensure that they comply with the club policy and procedures (available on the website).
- I agree to Sleaford Barge Netball club using photographs and video for use in training, advertising of events and PR through club website and social media.
- I understand in the event of injury or illness all reasonable steps will be taken to contact me, and to deal with that injury/illness appropriately.
- I give permission for their details to be kept in line with the Sleaford Barge Netball Club GDPR Privacy Policy.
- I understand that their personal details will be used to affiliate them to England Netball.
- I understand that my contact details may be used by the Club for Club business.
Emergency contact details will be sent to Coaches and a copy kept at training.
Please note, if you select 'no' as the consent field, then you (or your son/daughter/child) will not be able to participate in Sleaford Barge Netball Club.
